Work Sheet for the Israel Prophecy Study tour

March 14-27, 2012

1) x Please send a copy of the main page of your passport (the page containing all the information and your picture).

If you don’t have a passport, please call me and let me know so I can be working with you. And then proceed; here is what you need to do:

* Have passport photos (two) taken

* Have birth certificate (the original - with raised seal)

* Have to fill out the application (which can be obtained at your local post office)

* Have to have a check for $100.00 (this amount is subject to change)

* Have to take driver’s license

* Need to see an agent of the post office to validate and process this for me

* You should receive your completed passport back, by mail, within 4 to 6 weeks!

2) x Please send a copy of the itinerary sheet that you received with your domestic ticket (the ticket to and from the New York area).

* If you need me to do your domestic ticketing and have not already told me so, please let me know.

3) x Please send your policy number for the travel insurance, and sign the form that you have purchased travel insurance - if you have purchased that coverage. If you do not purchase travel insurance, please sign the waiver to that effect. And return that to me at Joshua Travel, ASAP. If you have any questions, please contact me.
